So, I have played through the first game many times. I have beaten Vet and Hardcore difficulties in the past, and have the achievements to show for it. I went to start a new game on Insanity. Set the difficulty on the main menu. Part the way through, it seemed too easy, so I checked the options, and I was on Vet. Both Hardcore and Insanity are locked again. I've tried all kinds of things, but it's still locked. I really don't want to play through two more times just to unlock Insanity again. Is there any way to fix this? Oh. Forgot to mention this is the Xbox version.

If you have a saved game where you finished the game on Hardcore, then deleting "profile.MassEffectProfile" (in Documents/My Documents folder) should force the game to recreate it and reward you with the relevant achievments and open that option. He said Xbox version, not PC. To OP I can't say I've ever encountered this glitch on my Xbox. If you are using the same console the original saves were made on I'm not sure where the problem could be. The only thing I could suggest is seeing if Insanity is available when starting a New Game, and if so do that. Also is this the disc or download edition of ME? It is the disc version. Same console, same everything. Been a while since I've played. I just picked up on an old save, got a couple achievements I was close to, then finished the game again. That was the character I wanted to use for Insanity and to carry over to the next games. So, on the main menu, I chose Insanity, then started a new game with that character. Part the way through Eden Prime, I checked, and the difficulty was set to Vet. Both Hardcore And Insanity are locked. Like I said, I even have the achievement for beating Hardcore. This whole thing is just strange, haha.
Indeed, and frustrating too I bet. Unfortunately the only thing I can suggest is perhaps looking on Bioware's own forums for any troubleshooting threads - If you're lucky someone else may have had the same problem and found a fix.
